Replacing eyeglasses or contact lenses can be expensive. Use your annual eyewear benefit to help save money. Get up to $150 toward the full retail price of frames, prescription lenses, a combination of frames and prescription lenses, and/or contacts from a provider in the EyeMed network, or $90 toward glasses and/or contacts from an out-of-network store.

$150 EyeMed network discount

More than 26,000 eye care providers make up the EyeMed network, including national chains like LensCrafters, Sears Optical, Target Optical, and JCPenney Optical. Present your member ID card when making a purchase, and $150 will be deducted from your total cost.1

$90 out-of-network discount2

After you make a purchase from an out-of-network eye care provider like BJ’s, Costco or Walmart, you can receive a $90 reimbursement by downloading an Out-of-Network Vision Services Claim Form from the “Forms” page of our website.3 You can also get the form by calling Member Services.

1Get up to $150 toward the full retail price (not sale price) of frames, prescription lenses, a combination of frames and prescription lenses, and/or contacts once every calendar year from a provider in the EyeMed network, OR up to $90 from a store not in the EyeMed network. Can be applied to complete pair of eyeglasses (lens, frames and lens options) and/or contact lenses. $150 discount cannot be combined with any other discounted offer. Sale items excluded and cannot be combined with store discounts, coupons or promotional codes.

2$90 out-of-network discount limit applies to HMO members only. PPO members can use the $150 allowance in-or out-of-network.

3The $90 out-of-network benefit cannot be combined with the $150 in-network benefit.
