There are several ways you can enroll in a CarePartners of Connecticut plan.
Enroll by phone:
Licensed Medicare Agents can answer your questions and take your enrollment form over the phone.
Call 1-844-267-1361 (TTY: 711) 7 days a week, from 8 a.m. to 8 p.m.
Enroll by mail:
Simply download and print the 2022 HMO enrollment form or 2022 PPO enrollment form or request a mailed copy by phone. Send your completed form to:

Enroll at a Virtual Meeting:
Attend a virtual meeting with a Licensed Medicare Agent and get your questions answered. Plus, as an attendee you can reserve a personal 1x1 virtual consultation with the presenter who will help you enroll in the plan that is right for you - from the comfort of home.
Enroll through Medicare:
Call 1-800-Medicare (1-800-633-4227) (1-877-486-2048 for TTY users), 24 hours a day, 7 days a week. Or, Medicare beneficiaries may also enroll in CarePartners of Connecticut through the CMS Medicare Online Enrollment Center located at http://www.medicare.gov.
