  3. $0 Monthly Premiums: Our Medicare Advantage HMO plans and and Access PPO plan are $0 a month. 
  4. Prescription Drug Savings: With Rx drug coverage included, our plans offer members great ways to save on their prescription drug costs such as:
    • $0 copay for Tier 1 and Tier 2 drugs1  
    • $0 vaccines on Tier 6, including Shingrix
    • Savings of up to $400 a year on prescription drug costs by using Optum Home Delivery

Up to $150 Eyewear Allowance: Members can get up to $150 toward the full retail price (not sale price) for eyeglasses, prescription lenses, frames, and/or contact lenses from a provider in the EyeMed Vision Care Network (includes more than 26,000 eye care providers, including national chains such as LensCrafters®, Pearle Vision®, and Target® Optical). Or get up to $90 from a store not in the EyeMed network. Discounts can’t be combined. (CarePartners Access PPO members do not have an EyeMed- participating provider requirement).
  6. SilverSneakers® Included: Our plans include a SilverSneakers benefit at no additional cost. SilverSneakers can help members improve health and independence. Whether members play tennis, swim laps, do yoga, or lift weights, SilverSneakers, which is part of our plan, has members covered.   7. Hearing Aid Savings: With copays ranging from $250 to $1,150 for each hearing aid, members can choose from 5 levels of hearing aids from Standard level to Premier. Members are covered for up to 2 hearing aids per year, 1 hearing aid per ear, provided by Hearing Care Solutions. Members are also covered for a $0 hearing aid evaluation once per year.2

  9. Dental Coverage that Covers More4: Our plans have dental coverage included coverage that can be used in- and out-of-network.4
    • CareAdvantage Preferred HMO—Includes $3,000 of dental coverage.
    • CarePartners Access PPO—New for 2024, members receive a Visa® Flex Advantage spending card with $1,200 a year to pay for non-cosmetic dental services at any dentist who accepts Visa—anywhere in the country.5
