Members Can Earn a $50 Gift Card for Completing an Annual Wellness Visit

CarePartners of Connecticut HMO and PPO members can earn a $50 Target or Walmart gift card by completing their Annual Wellness Visit in 2023. CarePartners of Connecticut members pay $0 for an Annual Wellness Visit with a network provider. 


Getting the reward is as easy as 1, 2, 3… 

  1. Members schedule an Annual Wellness Visit between 1/1/2023 and 12/1/2023. 
  2. Members bring the Healthy Reward Form to their Annual Wellness Visit to help guide the conversation with their provider. Members should have received a Healthy Reward Form in the mail (new members received a copy in their Welcome kit), but members can also print a copy of the form at carepartnersct.com/healthy-reward. 
  3. Upon completion of an Annual Wellness Visit, members fill out the Healthy Reward Form and return it postmarked by 12/1/2023 and receive their gift card within 4–6 weeks. 

All CarePartners of Connecticut HMO and PPO members can participate in the Healthy Reward program designed to promote preventive care.
