Deadline for Members to Get Up to $325 Back
Our Wellness Allowance (CarePartners of Connecticut CareAdvantage Preferred members only) and Weight Management benefit (CareAdvantage HMO members only) pays members back for participating in a qualifying program. But reimbursement forms for 2022 need to be received by CarePartners of Connecticut by March 31, 2023.
If a member enrolled in a fitness class, paid fees towards a gym membership, joined a qualifying weight management program, or participated in another qualifying program in 2022, they can be reimbursed by filling out and submitting these reimbursement forms:
- CareAdvantage Preferred members can get up to $175 back with our Wellness Allowance benefit¹ by submitting the Wellness Allowance Reimbursement Form
- CareAdvantage Preferred and Prime HMO Members can get up to $150 back with our Weight Management benefit² by submitting the Weight Management Reimbursement Form
Reimbursement forms for 2022 need to be received by CarePartners of Connecticut by March 31, 2023 – so encourage members to mail it as soon as possible!
