EyeMed and Your Annual Eyewear Benefit

How it works
Preferred HMO members get up to $300 and Access PPO members get up to $250 toward the full retail price of frames, prescription lenses, a combination of frames and prescription lenses, and/or contacts, including upgrades.
- More than 26,000 eye care providers make up the EyeMed network, including national chains like LensCrafters, Sears Optical, Target Optical, and JCPenney Optical. Present your member ID card when making a purchase, and your discount will be deducted from your total cost.1
- If you make a purchase from an out-of-network eye care provider like BJ's, Costco, or Walmart, you can receive a reimbursement up to your benefit amount by submitting the Out-of-Network Vision Services Claim Form from the "Forms" page of our website. You can also get the form by calling Member Services. In- and out-of-network discounts cannot be combined.
1You can get up to $300 (HMO) or $250 (PPO) toward the full retail price (not sale price) for eyeglasses, prescription lenses, frames, and/or contact lenses including upgrades. Only one purchase is allowed per calendar year up to the benefit amount; any unused amount after the single purchase will expire and cannot be applied toward another purchase during the calendar year. You can purchase from providers in the EyeMed Vision Care Network or from a provider not in the EyeMed network. If you use a non-EyeMed provider, you would need to pay out of pocket and submit for reimbursement. Discounts can’t be combined. Please refer to your Evidence of Coverage for more details.
